Dialogue Session – Collaboration in Practice: Strategies for Building Strong Payer-Provider Partnerships

Achieving improved patient outcomes at lower overall costs takes strong partnerships between a payer and the contracting provider organizations, to support the clinical workflow, and operational changes – and challenges – inherent in implementing a new payment model. Providers need financial and technical support to redesign clinical workflow and care delivery management systems if they are to succeed in improving outcomes while sustaining fiscal viability. This session will feature leaders from payer and provider organizations describing how their partnerships emerged and how these relationships and subsequent partnering strategies allowed APMs to succeed and in some cases, expand/scale to broader markets.
