202 Panel: Achieving Health Equity and Addressing Health Disparities via APMs

This session will focus on the concepts of health equity and reducing health disparities as they pertain to various consumer and patient communities, before talking about the importance of taking disparities into account when designing and implementing APMs. Panelists will include patients, providers, and payers speaking to the ways in which alternative payment can be used to address health equity by challenging the barriers that create disparities, delving into the specific strategies being used today, and aspirational strategies for the future.
