Panel – Payer-Provider Collaboration to Improve Outcomes for High Need Populations

High-need populations – whether they be patients with multiple chronic conditions and/or patients with co-existing physical and behavioral health needs – are most at risk for poor health outcomes due to lack of care coordination and care management. This session will feature experts sharing their experiences related to how to establish and scale care transformation teams, and what core functions care transformation teams should serve, to best support the most vulnerable patients. It will also feature payers who have developed systems to help providers identify patients who are currently high-risk, high need or who are at the cusp of falling into that category.
