Panel – Quality Measurement: “Big Dot” Population-Health Outcome Measures

Performance measurement is foundational to effective APMs, and there is consensus that the field must move from measure sets that are largely process-focused to measures that address outcomes of care and patient care experiences. More than three years ago, the LAN introduced the notion of “big dot” measures in its white paper on Performance Measurement in Population-Based Models. This session examines emerging, cutting edge approaches to using population-health outcome measures in APMs. In particular, it looks at the novel outcome measures being introduced in APMs, their connection to payment, and importantly, the ways they are encouraging providers to reach beyond the four walls of the clinic to address social determinants of health that support the achievement of better patient outcomes.
