204 Panel: Exploring Social Determinants of Health in Value-Based Care

This session will feature speakers who – recognizing the impact that social environment has on individuals’ health and the health of communities – are using innovative payment models to create reimbursement structures that account for critical non-clinical services that directly affect patients’ ability to access care (e.g. transportation support, care navigators, others). Panelists will highlight how organizations are collaborating—or could partner in the future—on integrated strategies within APMs and wrap-around support (transportation, food security, care navigators, and others) to manage social determinants of health to achieve better outcomes. This session will reflect the importance of addressing social determinants of health to be successful at bending the cost curve.
