2F: Listening Session: Primary Care Payment Model (PCPM) Recommendations (Repeat 5D)

This session will feature a discussion of the findings and recommendations included in the PCPM Draft White Paper which will be released prior to the Summit. Work Group Co-chairs Bill Golden and Susan Edgman-Levitan will review the Work Group’s recommendations for primary care payment model adoption and implementation. They will also describe the underlying assumptions and guiding principles used by the Work Group to develop their draft recommendations. In addition, they will share insights into APM implementation challenges considered throughout the PCPM Work Group’s iterative process. Perspectives will include purchasers, payers, providers, and patients.
